Bookmarks

Use bookmarks to automatically insert up-to-date data from the patient chart within SMART forms. Bookmarks are characters or a code that gets replaced with the corresponding patient data once the form is inserted into a patient's chart. For example, you add a book mark to automatically insert the patient's name, health number, and family MD from the patient's chart. See Adding bookmarks to SMART forms.

Several types of bookmarks are supported:

Bookmarks for which support is built in to Wolf EMR. These bookmarks are listed in the tables below. When inserting them into the document template, use the exact names listed below as the bookmark name.

Bookmarks that reference a built-in exam field. For these bookmarks, the bookmark name must start with "EX_", followed by the name of the exam field. For more information, see Adding bookmarks to SMART forms.

Bookmarks that reference custom exam fields that you added. For these bookmarks, the bookmark name must start with "EXC_", followed by the name of the custom exam field. For more information, see Adding bookmarks to SMART forms.

Tip: If you want to create placeholders within SOAP visit notes, letters, follow ups, and messages, instead use SMART tags.

To add more than one of the same bookmark, add a number at the end of a bookmark. For example, to include the consultant name twice within a Word template, insert the bookmarks ConsMDConsultant and ConsMDConsultant1.
