Cardiac risk worksheet

The Cardiac Risk Worksheet shows the patient's current cardiac risk based on information from their patient record. You can determine the patient's potential risk percentage of attaining CVD in the next 10 years based on the risk factors. You can change controllable risk factors to see how they would ultimately affect the patient's risk.

Within a SOAP note for the patient, click the Cardiac Risk link located on the right side.

In the patient's chart, right-click and choose Cardiac Risk Worksheet.

Heart age is an indicator of a patient's heart health based on the Framingham Heart Age Study. In the Framingham study, a patient has a current heart age based on current vitals and lifestyle and an optimized heart age which would be the optimal heart age the patient could achieve.

To calculate the current and optimal heart age, in the Modifiable Risk Factors section, modify a risk factor and press Enter on your keyboard or click the calculator button to see the potential impact on cardiac risk. You can play with these numbers to see how these changes can ultimately affect a patient's heart age.

Important: In the Modifiable Risk Factors, the original values are pulled from the patient record (latest vitals, lab values, and smoking history from the medical summary). You can change them for educational or information use only. Any changes you make here are not saved to the patient record.

As you play with risk factors, the heart age changes and the risk bar levels change to visually indicate the patient's risk percentages. Green indicates a low risk, yellow a moderate risk, and red a high risk.
