Using SOAP templates
A SOAP template is a version of the SOAP form that is pre-populated with information specific for a certain type of visit. SOAP templates can have:
The Chief Complaint field pre-populated with a visit reason.
History builder options customized to the visit type (see Using the subjective history builder).
Information pre-entered in any of the text fields.
A specific exam template open (see Using structured exam templates).
A high-level problem added to the Assessment field, such as diabetes.
Chronic Disease Management (CDM) SOAP templates
Wolf EMR contains numerous Chronic Disease Management (CDM) SOAP templates for you to use. The CDM templates structure the SOAP form around current disease-specific clinical guidelines.
CDM SOAP templates help you understand your patient base and focus on prevention when monitoring chronic disease patients. If you enter CDM visit data using appropriate SOAP templates, you can:
View a patient’s latest vitals and lab results on the visit note itself.
Track patient trends, disease progress, and compliance for entire groups of patients diagnosed with a specific chronic disease (using Practice Search reports).
Track a specific patient’s treatment plan, disease progression, and compliance (using disease-specific flowsheets).
Receive automated reminders for chronic disease patients who are due for specific tests, follow-up appointments, lifestyle advice, and treatments (using Rules).
Wolf EMR includes the following CDM SOAP templates:
Diabetes Mellitus Review
Diabetes Mellitus (BCMA)
Hypertension (CDM)
COPD
CHF (Cardiac Heart Failure)
Chronic Kidney Disease (CDM)
Depression (CDM)
Steps
1. | Open the SOAP form for the patient. See Starting SOAP notes. |
2. | In the Template Search list, click the template you want. The EMR loads the template in the SOAP form and displays the template name at the top of the SOAP form. |
Tip: If the list does not include the template you are looking for, you can configure your WorkDesk to show (or hide) additional ones. From the WorkDesk menu, choose Configure > Configure WorkDesk > Visit Exams tab.
3. | If the SOAP template is a CDM template: |
a) | In the SUBJECTIVE area, enter your notes using History Builder. See Using the subjective history builder. |
b) | In the OBJECTIVE area, enter your notes in the EXAM area. See Using structured exam templates. |
4. | Edit any other pre-populated information as necessary, and complete the remainder of the form as normal. See Entering visit notes in the SOAP form. |
