WorkSafeBC forms

You can complete and bill for the following WorkSafeBC forms from Wolf EMR.

Form name Description

Form 8: First Report

Used for the first occasion that a provider provides medical aid and reporting in relation to a work-related accident.

Submit this to WorkSafeBC if you think there may be time loss beyond the day of the injury or if the claim is for a hernia, back problem, shoulder/knee strain or sprain, or occupational disease.

See Creating WorkSafeBC first reports.

Form 11: Progress Report

Used for follow-up visit reports of injury after a Form 8 First Report was already submitted for the patient.

Used to report relevant changes since the previous report, such as a change in medical condition, treatment plan, or return to work status.

See Creating WorkSafeBC progress and other reports.

If your clinic is not on the data centre, use the forms on the WorkSafeBC site http://www.worksafebc.com/forms/.

Most forms are PDFs designed to be printed out and completed. Others can be completed and submitted online, and some are available as an online application. You can also order printed forms at WorkSafeBC Store.
