Creating care teams
If your clinic works in “teams” or “pods” of providers to manage groups of patients, you can create care teams in Wolf EMR. For example, a care team has two providers (Dr. Mack and Dr. Kiwi). If Dr. Mack is away and a patient who's on the care team books an appointment, front-office staff can see that this patient belongs to the Mack/Kiwi care team and can instead book the patient with Dr. Kiwi.
A provider can be on more than one care team. Only service providers can be on a care team; front-office staff cannot be assigned to a care team.
When you work with care teams you:
Create a care team and add providers to it.
Assign patients to a specific care team (in the Patient Maintenance window).
Produce practice searches that only include patients assigned to a specific care team.
Note: Care teams do not affect messaging or task groups. If you want to be able to send messages and tasks to a specific care team as a whole, create security groups for each care team. See Creating security groups.
Steps
1. | From the Wolf EMR home page, click Configuration ![]() |
2. | In the Configuration window, choose View > Physicians/Service Providers > Care Teams. The Care Teams window opens. |
3. | To add a new resident click . |
4. | Click New Care Team![]() |
5. | In the Name field, type the name of the care team (up to 10 characters). |
6. | In the Description field, type a description. This appears in the patient banner of the medical summary when a patient is assigned to the care team. |
7. | To add a user to the care team, click Add Member ![]() |
8. | To remove a user, in the Care Team list, click the care team on the left and the user on the right and, in the End Date column, enter an end date. |
Tip: To select a date from a calendar, click the down arrow in the End Date column.
9. | When you are done, click Save ![]() |
