Customizing the subjective history builder

From within the subjective history builder (see Using the subjective history builder), you can customize:

Opening statements (such as Patient complains of... and Patient denies... )

Systems (such as Cardiovascular, Eyes, Next, and so on)

Symptoms within each system

The changes apply for all providers in the clinic.

Steps

1.

Open the subjective history builder. From within a SOAP note, in the SUBJECTIVE area, click Open Hx Builder. Starting SOAP notes.

In the SUBJECTIVE area, click Open Hx Builder. The EMR expands the SUBJECTIVE area to display the Hx Builder area,

To edit the opening statements:

a)

Double-click in the opening statement area. The History Builder Opening Statements window opens.

b)

Edit the opening statement text or add a new one in the last row.

c)

If the statement represents a negative, select the Phrase is negative check box.

4.

To edit the systems:

a)

Double-click a system in the list. The History Builder System Review window opens.

b)

Edit the system text or add a new system in the last row.

5.

To edit the symptoms:

a)

Double-click a symptom. The Symptom Registry window opens.

b)

Edit the symptom text or add a new symptom in the last row.

c)

To change the order, type a value in the Order column, starting with 1. If no order is specified, they appear in alphabetical order.
