Adding custom patient demographic fields
If you need to store additional patient demographic information in patient records, such as mother's maiden name, you can define up to 24 custom fields.
Each field that you define appears on the Patient Maintenance window > User-Defined Fields tab. These fields can also be added to any user-defined patient forms that you define.
Steps
1. | From the Wolf EMR Home tab, click Configuration ![]() |
2. | From the View menu, choose Other > User-Defined Patient Fields. The User-Defined Patient Fields window opens. |
3. | To add a new field, in the upper-left corner, click the new button ![]() |

Type one of the following formats for the field: Text Numeric Date |

Enter the order that this field displays on the Patient Maintenance window. 1 appears at the top, followed by 2, and so on. |
4. | To edit a field, click it in the list and change its information. |
5. | To delete a field, click it in the list and from the File menu, click Delete. |
When you delete a field, all data that was entered in this field in patient maintenance is also deleted.
6. | When you are done, click Exit ![]() |
